All fellowship position openings are offered through the NRMP. All application materials must be submitted through Electronic Residency Application Service (ERAS).
Please submit your application and the following supporting documentation via the ERAS system:
- Curriculum Vitae
- ECFMG certificate (if applicable)
- Letters of recommendation – three (3), one of which should be from your Residency Program Director
- Medical school diploma
- Medical school transcript
- Medical Student Performance Evaluation
- Personal statement
- Photograph of yourself
- USMLE or COMLEX scores
AMSSM Code of Ethics should be emailed directly to the program coordinator.
Early submission of application materials is encouraged. Please see the ERAS timelines for deadlines. We are currently only accepting applicants who are completing or have completed a residency in family medicine.
Eligibility
The applicant must be currently enrolled in or have graduated from an ACGME-accredited family medicine residency program.
If the applicant is accepted and appointed to the Mercyhealth Sports Medicine Fellowship Program, the following are required prior to starting:
- Unrestricted Wisconsin medical license
- DEA
- BLS and ACLS certifications
- Immunization records
Please note that Mercyhealth will reimburse expenses for fellow's Wisconsin medical license, DEA license and other required certifications.
